Where do 'leaks' come from?

Perhaps I'm simply demonstrating my ignorance but who actually leaks albums? Who is it who possesses a copy of the album at hand to then create a torrent and then leak it? Is it people in the recording studio, or maybe someone who has managed to get hold of a copy for early review and then puts it out on the internet? Basically who is in the position to get hold of material long before it's release date and then put it on the web anonymously? Perhaps these sources are common knowledge but I find myself unclear nontheless.
