Advice Sought On CD Manufacturing

Greetings fellow cosomonauts,

We've just recorded a new set of songs that we intend to release as an EP and have looked at a couple of manufacturing companies with a view to running off some copies. Basically, it's the first real time we've been in this situation and we've no idea how you go about doing this, so that's where (hopefully) you guys come in....

1) Do you have to supply the cover/sleeve on a laminated piece of paper or will just a printout do?

2) If you wanted to use a black and white photo on the cover, do you give a small or large print of this photo to the company? Or do you just give them the negative or the JPEG?

3) Barcodes - we intend to have this CD stocked in shops, so presumeably that means we'll need a barcode, right?

I realise these probably sound like stupid questions to a few of you but we'd definitely appreciate any useful advice you could give.
