Please fill in my improbably boring and badly designed survey

I'm sure you don't care about the subject. I certainly don't. I was just considering a related issue earlier and it seemed like an easy option for my "Research Skills" (i.e. survey and report) class. There's absolutely nothing that requires thought on it, I assure you. Please note that failure to fill it in will result in me failing the course and not getting into university and dying. Probably. Kthnx.

And yes, I know that with it being split into two parts it defeats the point and makes the stats unusable. But I don't care. I just need a survey to print off and attach, and some general overall answers so that I can make up the rest. Awesome.
